The Seneca Nation Area Office for the Aging is pleased to offer NY Connects, a program that offers a Long-term and Supports (LTSS) system to individuals who desire to live as independently as possible and remain at home and in their communities. This program offers Information & Assistance (I&A), application and enrollment assistance for public health benefit individuals through financial and functional eligibility processes. Options Counseling/Person-Centered Counseling is also available.

NY Connects/No Wrong Door serves disabled individuals of any age by providing information and resources and assisting individuals and families to access services and/or programs. The goal is to provide the individual, family, and/or caregiver a seamless experience in accessing Long-term Services and Supports (LTSS).

Please call Gail Kennedy, Options Counselor at 716.945.8991. She will be pleased to assist you.
