The SN AOA participates with NY Connects: Choices for Long Term Care. The purpose of the NY Connects Program is to provide consistent, locally based information and assistance on long term services and supports (LTSS) to individuals, caregivers and families to help them make informed choices and assistance in accessing such services and supports. Additionally, the NY Connects: No Wrong Door network offers consistent comprehensive information, assistance and coordinated guidance to available public benefit options, as appropriate, regardless of age, disability or payor source. It is an optional, voluntary service provided to the public to create seamless coordination across systems and populations seeking LTSS and provide easier access and understanding.
